Collaboration among healthcare providers is straightforward in an acute or ambulatory care setting. After all, nurses, physicians, therapists and social workers typically see patients in one location, each having the opportunity to meet face-to-face and discuss the patient’s care. In home health and hospice care settings, however, it is more difficult to create a community among providers because communication and collaboration challenges are amplified since many providers are rarely in a traditional care setting-or a patient’s home-at the same time.
